- •The 4th IWGP Heavyweight Championship belt (V4) debuted on March 30, 2008 when champion Shinsuke Nakamura first wore it defending against Hiroshi Tanahashi at Korakuen Hall.
- •The V4 design features a large, rounded gold centerplate with lion engravings, a prominent globe at the top, and a distinctive red leather strap, with side plates listing former champions.
- •Kazuchika Okada's record 720-day reign (2016–2018) — the longest in the title's history — was carried entirely on this 4th-generation belt design.
- •The V4 belt was retired on March 4, 2021 when Kota Ibushi unified the IWGP Heavyweight Championship with the IWGP Intercontinental Championship, but was restored in January 2026 by Yota Tsuji.
